Student group insurance

 

Q1: Where can I get the Student Group Insurance Claim Application Form?

A1: You can get the application form from Health Center.

Q2: How to apply for the insurance compensation of Student Group Safety Insurance when a student is injured accidentally? 

A2:

  • Applying the compensation of an accidental injury:
  1. 1 application form
  2. 1 original copy of medical certificate
  3. original receipts (duplicates shall be certified by the hospital official seal and marked "identical as the original".
  4. In case of bone fracture, an X-ray film stored on a CD disk is required.
  • Death benefit payments:
  1. 3 copies of applications (each application shall fill in the relevant information of the legal beneficiaries and be stamped; surviving children shall fill out and stamp the applications; the bank accounts of the beneficiaries shall be provided in the form for wiring payments).
  2. 1 original copy of death certificate.
  3. 1 original copy of certificate of household registration (full version) with the deceased delisted.
  4. 1 copy of the student identification issued after the completion of the semester registration of the academic year at the time of death.
  • Disability benefit:A disability certificate issued by a hospital or accredited clinic 180 days after the date of accident.

Q3: If I have been insured by my own life insurance, can I still apply the insurance compensation of "Student Group Safety Insurance"?

A3: Even you have already insured by other life insurance, you still can apply for the claim of payment of "Student Group Safety Insurance". Submit the original medical diagnosis certificate and various original receipts of medical expenses, or duplicates of receipts stamped with the hospital official seals for processing the claims.

Q4: If I am in suspension, do I still have the coverage of "Student Group Safety Insurance"?

A4: When a student has completed the procedure of suspension or graduated already, the "Student Group Safety Insurance" still take effect until the end of that semester (the first semester ends on January 31st; the second semester ends on July 31st). The method and procedure for the insurance claim is the same as students during enrollment

 

Freshmen Health Examination

 

Q1: Do all the freshmen must take the health examination? 

A1:Yes, all the freshmen must take the health examination. In order to provide the  best care for the students of WENZAO, the health center arranges freshmen health  examination according to the regulations stipulated by the Ministry of Education.

Q3: What are the items of exam?

A3:

  1. General Exam: Height, weight, pulse, blood pressure, eye sight, oral cavity and pathological exams.
  2. Blood Test: Complete Blood Count (CBC), Hepatitis B (HBsAg, HBsAb), Liver   function (GOT, GPT), Urin Acid (UA), Renal function (BUN, Cr.), AC Sugar,   Triglyceride (TG), Cholesterol (T-CHOL).
  3. Urine Test: Urine protein, urine sugar, occult blood, PH.
  4. X-ray Exam: Chest X-ray

Q4: Without participating in the health examination, what shall the student do?

A4: The student can bring the " Student Health Examination Form " to any qualified hospitals to complete the health examination before the deadline and mail the examination card to the  Health Section.

Q5: What would I do if I had already done my own health examination on a regular basis?

A5: If the following 2 requirements are met:

  1. The health examination is performed during the year of school admission
  2. The items performed in the health examination are included all items listed on the back of the "Student Health Examination Form"; then, the student does not need to participate the health examination held by the university.
